These Terms and Conditions apply to your use of this website and, where relevant, to services provided by F3 Optimising Health (“F3”, “we”, “us”, “our”).

By using this website, contacting us, booking an appointment, completing a health questionnaire, attending a consultation or purchasing a service from us, you agree to these Terms and Conditions. If you do not agree with them, please do not use this website or book services with us.

1. About F3

F3 Optimising Health Urban Clinic provides private health optimisation, lifestyle medicine and medical consultation services. Our approach combines clinical assessment, prevention-focused care, lifestyle medicine principles, functional medicine-informed assessment and practical personalised support.

F3 is operated by F3 Optimising Health, registered in England and Wales under company number 17392731. Our registered office is 27-29 Ashley Road, Bristol, BS6 5NJ.

Where required, services are provided under the relevant regulatory, professional and clinical governance arrangements. Our CQC registration details, where applicable, are:

2. Medical emergencies

F3 is not an emergency medical service.

If you think you may be experiencing a medical emergency, including chest pain, severe breathlessness, symptoms of stroke, loss of consciousness, severe allergic reaction, severe bleeding, suicidal crisis or any other urgent or life-threatening problem, you must call 999 or attend your nearest Accident and Emergency department immediately.

For urgent but non-life-threatening medical advice, contact NHS 111, your NHS GP, an urgent treatment centre or another appropriate urgent care provider.

Do not use this website, email, online forms, social media or appointment booking routes for emergencies.

3. Website information is not a substitute for medical advice

The information on this website is provided for general information only. It is not intended to replace a consultation with a suitably qualified healthcare professional.

Website content may refer to health optimisation, lifestyle medicine, nutrition, physical activity, sleep, stress, functional medicine-informed assessment, blood tests or other health-related topics. This information is general in nature and may not be suitable for your individual circumstances.

You should not start, stop or change any medication, supplement, diet, exercise programme or treatment plan based only on information on this website. Always seek individual advice from an appropriately qualified healthcare professional.

4. Use of this website

You may use this website for lawful personal use only. You must not:

You may view and download website content for personal, non-commercial use only. You must not reproduce, distribute, adapt, publish or commercially exploit any content without our prior written consent.

F3 may provide services including, but not limited to:

  • private GP or medical consultations;

  • health optimisation appointments;

  • lifestyle medicine consultations;

  • Health MOT assessments;

  • review of health questionnaires;

  • review of symptoms, risk factors and health goals;

  • blood tests or other investigations where clinically appropriate;

  • personalised health plans;

  • nutrition, movement, sleep, stress and wellbeing advice;

  • referrals or signposting to other services where appropriate.

The services available may change over time. Some services may be subject to eligibility criteria, clinical suitability, professional judgement, regulatory requirements or availability.

9. Age and eligibility

Unless clearly stated otherwise, our services are intended for adults aged 18 and over.

We may decline to provide services where we believe the service is not clinically appropriate, where your needs would be better met by another provider, where emergency or specialist care is required, or where we are unable to provide care safely.

10. Booking appointments

Appointments may be booked online, by telephone, by email or through another approved booking route.

When booking, you agree to provide accurate and complete information. This may include your name, contact details, date of birth, relevant medical history, medication, allergies, GP details, emergency contact and other information needed to provide safe care.

An appointment is confirmed only once we have accepted the booking and, where required, received payment or deposit.

We reserve the right to refuse, cancel or rearrange appointments where clinically necessary, operationally necessary or where the service requested is unsuitable.

11. Appointment format

Appointments may take place face to face, by telephone, by video consultation or through another agreed method.

Not every concern is suitable for remote consultation. If we consider that you need a face-to-face assessment, urgent review, physical examination, emergency care or specialist care, we will advise you accordingly.

Remote consultations depend on accurate information being provided by you. We are not responsible for clinical limitations caused by incomplete, inaccurate or withheld information.

12. Clinical assessment and professional judgement

All clinical advice, investigations, prescriptions, referrals and recommendations are subject to professional judgement.

A consultation does not guarantee that you will receive a prescription, test, referral, fit note, letter, diagnosis or treatment requested by you. We will only provide these where clinically appropriate and within the competence, scope and regulatory responsibilities of the clinician involved.

We may recommend that you contact your NHS GP, NHS 111, A&E, a specialist service or another healthcare provider where this is clinically appropriate.

13. Health optimisation and functional medicine-informed care

F3 may use a functional medicine-informed and lifestyle medicine approach to understand broader factors that may affect health, energy, resilience and wellbeing.

This approach does not replace conventional medical assessment, diagnosis or treatment. Where symptoms suggest a medical condition requiring investigation, treatment, urgent care or specialist input, we will advise you accordingly.

Health optimisation plans are intended to support wellbeing and prevention. Individual results vary and we do not guarantee specific outcomes.

14. Medication and prescriptions

Prescriptions are issued only where clinically appropriate and at the discretion of the prescribing clinician.

We may decline to prescribe medication where:

  • it is not clinically appropriate;

  • further assessment is needed;

  • monitoring is required that we cannot safely provide;

  • specialist input is required;

  • the medication should be managed by your NHS GP or specialist;

  • there is a risk of misuse, dependency or unsafe interaction;

  • the request falls outside our prescribing policy.

Private prescription charges, pharmacy dispensing charges and medication costs are separate from consultation fees unless clearly stated otherwise.

15. Tests and investigations

We may recommend blood tests, biomarkers or other investigations where clinically appropriate.

Tests may be provided directly by F3 or through an external laboratory, phlebotomy provider, imaging provider or other third party. Where a third-party provider is used, their own terms, policies and charges may also apply.

The cost of tests may be separate from consultation fees unless clearly stated otherwise.

Test results must be interpreted in clinical context. Abnormal, borderline or unexpected results may require follow-up, repeat testing, referral or urgent NHS review.

16. Sharing information with your NHS GP or other professionals

For safe care, it may be appropriate to share relevant clinical information with your NHS GP, consultant, pharmacy or other healthcare professional.

We will usually seek your consent before sharing information, except where disclosure is required by law, necessary to protect you or another person from serious harm, or otherwise permitted under applicable professional or legal obligations.

You are responsible for informing us of relevant existing diagnoses, medication, allergies, investigations, treatment plans and any advice given by your NHS GP or other professionals.

17. Fees and payment

Fees will be displayed on the website, provided during booking or confirmed before the service is delivered.

Payment may be required in advance, at the time of booking or on the day of appointment. We may refuse to provide non-urgent services where payment has not been received.

Fees may include consultation time, administration, review of pre-consultation information and preparation where stated. Additional fees may apply for:

  • blood tests;

  • laboratory analysis;

  • prescriptions;

  • medication;

  • letters or reports;

  • referrals;

  • extended appointments;

  • follow-up consultations;

  • cancellation or non-attendance.

All prices are subject to change. We will confirm the applicable fee before you commit to a paid service.

18. Cancellations, rearrangements and non-attendance

Please tell us as soon as possible if you need to cancel or rearrange an appointment.

Unless otherwise stated at booking, the following cancellation policy applies:

  • cancellation more than 48 hours before the appointment: no cancellation charge;

  • cancellation between 24 and 48 hours before the appointment: 50% of the appointment fee may be charged;

  • cancellation less than 24 hours before the appointment: the full appointment fee may be charged;

  • failure to attend: the full appointment fee may be charged.

We may waive cancellation charges at our discretion in exceptional circumstances.

If we need to cancel or rearrange your appointment, we will offer an alternative appointment or refund any fee paid for the cancelled appointment.

19. Refunds

Refunds are considered in line with the service purchased, the timing of cancellation and whether clinical or administrative work has already started.

Consultation fees may not be refundable where an appointment has taken place, where clinical review has begun, or where you have failed to attend.

Fees for tests, laboratory work, third-party services or specially arranged services may not be refundable once they have been booked, processed or commissioned.

This does not affect your statutory rights.

20. Online purchases and consumer cancellation rights

Where services are purchased online or at a distance, consumer cancellation rights may apply.

If you ask us to begin providing a service before the end of any applicable cancellation period, you agree that you may be charged for services already provided, including clinical review, administration, appointment time, tests or third-party costs.

Some services may be exempt from cancellation rights once fully performed or where they are time-specific appointments. You should seek independent advice if you are unsure about your consumer rights.

21. Memberships, packages or programmes

Where F3 offers memberships, packages, Health MOTs or programmes, separate terms may apply. These will be provided before purchase.

Unless clearly stated otherwise:

  • unused elements of a package are not transferable;

  • packages must be used within the stated period;

  • clinical suitability is assessed throughout the programme;

  • we may recommend stopping or changing a programme if clinically appropriate;

  • outcomes are not guaranteed.

22. Patient responsibilities

To help us provide safe and effective care, you agree to:

  • provide accurate and complete information;

  • tell us about medication, supplements, allergies and relevant medical history;

  • tell us if you are pregnant, trying to conceive or breastfeeding;

  • tell us about relevant mental health concerns or safeguarding risks;

  • follow urgent safety advice where given;

  • seek emergency or NHS care where advised;

  • attend appointments on time;

  • treat staff and clinicians respectfully;

  • pay fees when due.

We may decline or stop providing non-urgent services if a patient behaves abusively, threateningly, dishonestly or in a way that prevents safe care.

23. Safeguarding and risk

If we become concerned that you or another person may be at risk of serious harm, abuse, neglect or exploitation, we may need to share information with appropriate services in line with safeguarding, legal and professional obligations.

This may include contacting emergency services, your GP, safeguarding teams or other relevant professionals.

24. Complaints and feedback

We welcome feedback and take complaints seriously.

We will acknowledge your complaint and explain the next steps. We aim to investigate complaints fairly, promptly and transparently.

If you remain dissatisfied after our response, we will provide information about relevant escalation routes, which may include an independent complaints adjudication scheme if applicable, the relevant professional regulator or the Care Quality Commission for concerns about regulated care standards.

25. Privacy and personal data

We process personal data, including health information, in accordance with applicable data protection law.

27. Email, online forms and communication

We aim to respond to enquiries within a reasonable timeframe, but we do not guarantee same-day responses.

Clinical advice is only provided through an appropriate consultation or agreed clinical communication route.

28. Limitation of liability

Nothing in these Terms and Conditions excludes or limits liability for death or personal injury caused by negligence, fraud, fraudulent misrepresentation or any liability that cannot be excluded by law.

Subject to this, we are not liable for:

  • loss arising from misuse of this website;

  • reliance on general website content instead of individual clinical advice;

  • loss caused by inaccurate or incomplete information provided by you;

  • third-party website content or services;

  • website downtime or technical failure;

  • indirect or consequential loss where exclusion is permitted by law.

29. Changes to these Terms and Conditions

We may update these Terms and Conditions from time to time. The updated version will be posted on this website with a revised “last updated” date.

Your continued use of the website or services after changes are posted means that you accept the updated Terms and Conditions.

30. Governing law

These Terms and Conditions are governed by the laws of England and Wales.

Any dispute arising in relation to this website or our services will be subject to the courts of England and Wales, unless mandatory consumer protection law provides otherwise.
